This service agreement is entered into and effective as of your first session by and between, {name-1-first-name} {name-1-last-name} (here and after referred to as you/your) and Lindsay Hull, Soulful surrender (here and after referred to as I/me/my).
In entering into this contract to work together, we both agree to the following:
● SCOPE OF WORK
MindBody Coaching
One on One Session via Zoom: I will meet you where you are and help you create a practice that suits your needs and lifestyle. My coaching may entail practices such as: somatic practices, belief work, mindset work, inner child, visualization, journaling, etc. My intention is to support your individual needs and to keep you motivated and on track. As your coach, I will also guide you to different resources to help you cultivate safety and regulation in the nervous system.
This Service includes:
– 1 Hour session at the rate of $100 USD/hour via zoom.
– Email recap on session with main points and homework within 24-48 hours.
– One check in via email or text in between sessions. This coaching is different from counseling, psychotherapy, or psychoanalysis and does not deal with the diagnosis or treatment of problems.
I expect you to follow up with your doctor if you feel it’s necessary. This work can be done in conjunction with medical interventions. I recommend and advise you to continue seeing your licensed health care providers and continue following their advice and directions, including prescribed medications and directions, including prescribed medications and treatments. I DO NOT recommend you discontinue either, nor that you stop working with your licensed health care providers. Since coaching does not constitute medical consultation or treatment, health insurance does not apply.
● COMPENSATION
You will provide payment via PayPal, Stripe or e-transfer upon booking the session.
● CREATING IDEAL CONDITIONS
I ask that you commit to this work and show up ready and willing to take steps towards your health and healing. I am here to guide you, motivate you and support you as you heal. I ask that you find yourself a quiet spot with limited distractions so that we can get the most out of our time together.
● RESCHEDULING
24 hour notice is required in order to receive a refund of payment.
● GUARANTEE
I do not make any guarantees about the results of the information applied in one on one sessions. I share educational and informational resources that are intended to help you succeed in regulation. However, your ultimate success or failure will be the result of your own efforts, your particular situation, and innumerable other circumstances beyond our knowledge and control.
● CONFIDENTIALITY
I AM COMMITTED TO YOUR PRIVACY
Information revealed during consulting sessions (verbally, in recorded calls, or in written format) will remain confidential unless disclosure is required by law. These disclosure conditions include any reasonable suspicion of child, dependent, or elder abuse; when you are in danger to others; or when you are likely to harm yourself unless protective measures are taken.
PUBLIC DISCLOSURE: Neither party may disclose the terms of this Agreement. Neither party shall make any formal or informal public statement, press release or other announcements regarding the existence or terms of this Agreement without the other party’s prior written approval.
● NON-DISPARAGEMENT
You are agreeing that, during and after our contracted time together, you will refrain from making any statements or comments of a defamatory or disparaging nature to any third party regarding me, Lindsay Hull, Soulful Surrender, or any of my services.
● DISCLAIMER
All information presented in this website is for informational purposes only. This website and the services discussed are not intended to diagnose, treat, cure or prevent any disease. Content is not a substitute for professional psychological, psychiatric, or medical advice, diagnosis, or treatment. ● LIABILITY
By participating in a session or engaging with social media content, you acknowledge that you have read, understood and agreed to the following: You understand and acknowledge that Lindsay Hull is not providing medical advice and that the website, social media accounts, groups, or one on one sessions are not a substitute for working with your licensed healthcare providers. You understand that nothing presented in the website, social media accounts, groups or one on one sessions is to be taken as professional medical advice and is not a substitute for counselling, psychotherapy or psychoanalysis. It does not deal with the diagnosis or treatment of conditions or symptoms. It does not deal with specific trauma and is not a substitute for one on one professional support. You understand that the website, social media accounts, groups, one on one sessions are intended to be used as a compliment to your current healing practice and may provide general knowledge and skills to be used alongside your current practice. You understand that the guided exercises in the membership and one on one sessions are to be followed to the best of your judgement and that you are fully responsible for your own well-being. You understand that there are no guarantees with regard to the outcome of the membership and one on one sessions and that Soulful Surrender does not make any claims to healing or recovery as a result of these. You understand that the website, groups, and one on one sessions are voluntary and that by signing up you personally assume all liability for any injuries, losses of any kind, risks or damages, known and unknown.
● SOVEREIGNTY OF AGREEMENT
This Agreement constitutes the entire agreement between the parties and supersedes all prior and contemporaneous agreements, representations, and understandings of the parties. No supplement, modification or amendment of this Agreement shall be binding unless executed in writing by all the parties. No waiver of any of the provisions of this Agreement shall be deemed, or shall constitute, a waiver of any other provision.
● SEVERABILITY
If any term, provision, covenant or condition of this Agreement is held by an arbitrator or court of competent jurisdiction to be invalid, void or unenforceable, the rest of the Agreement shall remain in full force and effect and shall in no way be affected, impaired or invalidated.
This contract must be signed 24 hours before our next session.
